Terms and Conditions:

1) Registration fees are nonrefundable.  There are no tuition refunds after the first class. Credit towards future classes may be issued in unusual circumstances at the discretion of the director. 

2) A makeup week is added at the end of the semester for anyone who has missed a class for any reason.  Makeup slots are also available in other classes during the semester, depending on class size. Please sign up in advance for a makeup in a different class. 

3) We realize it is very tempting to want to document all the fun and musical growth your child may be having in every class, especially to show parents and grandparents who can't be there, but we respectfully ask that you put away your phone and/or camera during class.  There will be an opportunity for a "photo day" when you can take pictures and brief videos in the final class of the semester.  

4) We ask that you use common sense when it comes to illness and keeping others healthy. Please do not bring your child to class if he/she has had a fever, has been vomiting within the last 24 hours, or has diarrhea.  If you or your child have any symptoms of Covid, or have been instructed to quarantine or isolate because of Covid or exposure to Covid, please do not attend class. 

ASSUMPTION OF THE RISKS AND WAIVER OF LIABILITY:

As a willing participant in classes at Lori’s Music Together^®, my family and I agree to the following:

ASSUMPTION OF THE RISKS: I acknowledge that activities such as this have inherent risks. Although precautions will be taken, it is not possible to ensure the safety and health of participants and it is ultimately my responsibility to ensure that I and my child take appropriate actions to safe-guard ourselves. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself or my adult caregiver caregiver/representative, including, but not limited to, personal injury, disability and death, illness, damage, loss, claim, liability, property damage, or expense, of any kind, that I or my child(ren), or caregiver/ representative may experience or incur in connection to with our attendance at a Lori’s Music Together class or participation in any program held by Lori’s Music Together.

WAIVER OF LIABILITY: I, on behalf of myself and my child hereby voluntarily release, forever discharge, and covenant not to sue Lori’s Music Together, and its owners, managers, members, employees, agents and volunteers (“Released Parties”), and the owners, agents, and employees of the facilities and locations in which classes are held, of any claims that may arise out of or relate in any way to my or my child’s participation. The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties whether an injury or illness occurs before, during, or after participation in any Lori’s Music Together class or program.

I HAVE CAREFULLY READ THIS WAIVER AND FULLY UNDERSTAND ITS CONTENTS. BY ENROLLING IN CLASSES AT LORI’S MUSIC TOGETHER, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT
